Faculty Grade Report Form

Honors Grade Report Forms are due the Monday that course grades are due for the semester. Extensions must be approved by the Honors Director.

If you prefer, a paper grade report form also can be printed and completed in lieu of the web form and mailed, faxed or emailed to the Honors Program.

Please be sure to provide the grade the student received for the course AND the grade the student received for the Honors component.
