USI Parking may issue temporary disability parking permits to persons with short-term physical disabilities or medical conditions restricting their mobility. In order to obtain a Temporary Disability Parking Permit, a copy of the Practitioner’s Certification for Temporary Disability Parking form must be completed by the person’s physician or health care provider and then brought, faxed or mailed to the Parking or Public Safety office.
All temporary parking permits shall be issued for one, non-renewable 30 day period. If the disability/condition restricting mobility is expected to last longer than 30 days, the individual will be required to obtain a state-issued disability plate or placard from the Bureau of Motor Vehicles.
Completed form may be faxed by the office of the physician or health care provider to the USI Parking office. Once approved, the temporary disability permit can be picked up from 8:00 a.m. until 3:00 p.m. Monday through Friday; the fax number is 812-465-1279.
