External Grant Forms
These forms are used for External Grant Applications only, not internal awards, such as Endeavor! or FRCWG.
Grant Application Pre-Notice Form - This online form is used to notify OSPR of the intent to apply for External Grant funding. It must be submitted to OSPR at least 15 days prior to the grant submission deadline. This initial contact with OSPR allows for USI grant administrators to create the application package and route for approval in Caysue 424. Here is a preview of the Pre-Notice Form.
Faculty Workload Assignment Policy - This policy is used to determine where a PI/PD will devote effort on a sponsored project if/when awarded.
Faculty Workload Assignment Form - This form is completed with each external grant submission. This form must be completed and uploaded into Cayuse prior to the electronic routing process.
Administrative Workload Assignment Form - Used to determine where an administrator will devote effort on a sponsored project if/when it is awarded. This form must be completed with each external grant submission and uploaded into Cayuse prior to the electronic routing process.
USI Investigator Significant Financial Interest Determination Form - This form is used to determine if a PI/D may have an actual or potential financial conflict with an external sponsored project. This form must be completed with each external grant submission. The form must be uploaded into Cayuse prior to the routing process.
