Group Life and Accidental Death and Dismemberment 

The University provides a group term life insurance plan with accidental death and dismemberment insurance plan to eligible employees upon hire. There is no cost to employees; USI pays total premium.

This benefit is 1 1/2 times annual salary with maximum coverage of $100,000.

Reduction of benefit

Under this plan, your coverage amount reduces to 65% at age 66.

Beneficiary Designation

Active employees should designate their beneficiary through the Benefitfocus Website. While retirees can utilize the beneficiary form.
