All Clinical Experiences
  1. Meet with preceptor. Provide the preceptor and/or office manager with information located in the Preceptor Packet. Introduce yourself. Explain the need for clinical experience relative to your specialty.
    1. Provide the preceptor and/or office manager with your resume/CV which has been updated each semester to reflect your education and experience.
    2. Review the contents of the Preceptor Packet with the preceptor and/or the office manager.
    3. If the preceptor indicates a willingness to provide this educational service complete the Preceptor Agreement, Site Survey, and Initial Request for Educational Agreement (MOU/Affiliation Agreement) with the preceptor or office manager.
  2. Leave the Preceptor Packet with the preceptor, asking that the Preceptor Agreement and the Request for Education Agreement also known as (Memorandum of Understanding) be signed and returned to the Nursing Program. Inform the preceptor that the clinical experience cannot begin until this from is completed and returned to the Graduate Nursing Program.
  3. Email the completed Request for Education Agreement (also known as Memorandum of Understanding), Preceptor Agreement and Site Survey to USI1Nursing@usi.edu

Communicate to the course faculty and/or the graduate clinical liaison if there is no success in a preceptor or and Education Agreement (MOU) contact at least 4 weeks prior to the beginning of the course/semester.

DO NOT INITIATE CLINICAL PRACTICE until you have received confirmation by email that the appropriate paperwork has been returned and has been accepted by the program. The Preceptor Agreement, Clinical Site Survey, and the Education Agreement (aka Memorandum of Understanding) must be approved before you will be allowed to participate in a clinical experience as a student at the University of Southern Indiana, Graduate Nursing Program.
