What is CastleBranch?

CastleBranch is an online system that completes background screening, and tracks drug testing, immunization records, and other documents for students that have been accepted into the MSOT Program at USI. Click here to go to CastleBranch's website.

How do I know what I need?

Students will be notified by the program as to what package to order from CastleBranch.

The following items are required for all students enrolled in the Occupational Therapy Program professional level coursework:
  • Hepatitis B 
  • Hepatitis B Titer
  • CPR Certification
  • Physical Examination
  • Influenza
  • Tetanus, Diphtheria & Pertussis (Tdap)
  • Varicella (Chicken Pox)
  • Measles, Mumps & Rubella (MMR)
  • Tuberculois (Two-Step TB)
  • Medical History
  • OSHA 
  • HIPAA
  • Health Insurance
  • Drug Test
  • Background Test
